we know of at least a few holed/plugged coins that are in top-tier slabs w/out the problem having been caught.

have you ever encountered a ncs or anacs net-grade slab that says the coin was holed &/or plugged, when in fact it was'nt?

That would be a neat item to see. I've not heard of one.

When ANACS first began the "net" grading, just about any coin- not matter the detail grade- received a net grade of PO01 because of the hole. Now I see net ANACS holed coins with higher and higher net grades.

I don't think that's right. Any coin with a hole should be net all the way down to 01.
