1st PCGS submission. Help?

I'm sending in my free submissions and have never filled out the PCGS form,
For the DESIG. field, the example gives MS. Is this just to distinguish between MS and PR coins? Like even if the coin I'm sending in isn't in Mint State, it's still a business strike.
And, what exactly do they mean by, "To validate the Guaranteed Service, the name of the service must be clearly marked on the outside of the package" Does this mean I just have to have "PCGS" written on it, or something else?

For "name of service" they need to know what tier you are using (15 day, economy, etc.) - they want it on the outside of the package so they don't have to open the package right away, in order to find out and sort it according to turnaround times.
It's ok to take the designation (FT, or whatever) into consideration when deciding what PCGS coin number to fill in - they can always change that if they disagree with you.
