There have been a couple cases where teammates had the same last name and first initial, causing their first names to be put on their jerseys. I'm not sure what happened when two Mike Smiths were on the same team a few years ago.
I can't remember the player who complained, but one Oakland Raider in the late 1970s wanted his first name on his jersey and complained to the coaches about it. They told him that no one had both names on their jersey, and he responded that Eeghen did. The coach had to explain to him that the player's first name was Mark, and Van was part of his last name.
