We know that mint employees have purposely made error coins. Do you know of any proof coins that were made by mint employees acting on their own?

1970-S proof quarter struck on a 1900 Barber quarter. Many other proof "errors" were made and smuggled out in the late 60's early 70's.
Class II and Class III 1804 dollars, 1851-O proof dollar, many others from the 1858 - 1871 period.

Several Branch Mint Morgans apparently. Wayne Miller reported an 1887-O as a definitive no doubt proof, however NGC (and I think PCGS) declined to certify it. Some others in Carson City too, if I remember correctly.
