Complimentary submission question

IS there a form required for Gold Collectors Club submissions?

I ask here because:

a) searching on complimentary yields nothing on psacard.com
b) searching on free gets me Bill Freehan
c) I tried sending to info@psacard.com and got mail back saying "are you sure your
card was charged?" - my reply to that said you didn't answer my question, reply to me
said sorry I get 100 emails a day, reply to that got no answer
d) tried asking info@psacard.com again hoping I'd get someone else and got no answer.
e) The forms all say NOT FOR COMPLIMENTARY SUBMISSIONS

Surely you guys know?

1967and 1973 Topps baseball wantlists (any condition) welcome. Once had the #14 ATF 1967 set. Yet another collector like skylaneflyer, gimel1 who made it to the completion of 1967 only to need the money more than the company of 609 close friends.
Looking for oddball Norm Cash and Cleon Jones stuff, and 1956 team cards

    1967topps> Are you talking about the complimentary submissions included with a Gold (or Platnium) membership? If so, those form(s) are mailed to you and are included with your welcome packet. Normally you should expect to see this 7-14 days after signing up for a membership.

    If I remember correctly, there is a special form provided to you for your free club submissions. You should get it with your enrollment pack. If you didn't get it, I would call them and ask for one.

    The form has "C.C. Submission Form" in red block letters on it. It also has only six or ten spots to enter cards, depending on what level you purchased. It has to be on that form.
  • Then I have a question- how do you submit a complimentary submission?- I mean the type that you receive when PSA doesn't meet the guaranteed turnaround time. Do you do it on a regular form and include the voucher?

    If you are using vouchers, fill out the normal form and submit your vouchers as part of the payment. Make sure that the cards you are sending in fit the basic requirements for the service level the voucher is for.

    My voucher was for a Super Express service on a SINGLE T3 (20 bus. day turnaround). It took 27 days and PSA issued a voucher.
    The voucher says valid for a complimentary "T3 grading" service.

    My question is- the service I bought was Super Express, not "T3 Grading". And since there have only been 73 T3's ever graded, the odds of me getting another one of these is slim.

    Could I use the voucher for a Super Express 2 day turnaround for a single card since this is what I originally paid for?
