PCGS grading/submission question

I am sending some coins to PCGS next week (gold member). Question is, what do they consider a GSA 78 CC dollar. It is in a cracked holder so I need it placed in a PCGS holder, with the GSA designation. Do they consider this a cross over, a raw coin, etc? You have to list the raw, crossovers, etc on seperate forms. I am sending both crossovers and raw coins in and do not know what form to list this one on.
