I don't have a pic either, but I think there is one in the Cherrypicker Guide that preceded the current Vol. 1, and some day Vol. 2 (it should be in Vol. 2, whenever it conmes out).
I've never found opne of these in a proof set, and have only seen about two that I can recall- both slabbed. One was a PR66, possibly CAM(?). Interestingly, the dealer (at least I think it was the same dealer, I going back to the early 90's on this recollection) that had that coin also had NGC MS64 1916 DDO Buffalo Nickel.
