I just check the "other" box in the Service section on the form, and write in what I want them to do. For example, "FB Review" or "FBL Review", etc. I also fill in the fee as $10. There are sometimes posts that the fee is $5, but it is actually $10- I've asked multiple PCGS people on this one. (It was $5 several years ago).
