PCGS Crossover question

I am submitting my first four coins to PCGS and am crossing over two coins. One was an NGC, and the other ANACS. Both coins were cracked out long ago and all I have are the tags.
Does PCGS recognize those coins as legitimate crossovers now that they are submitted outside of the holder? If so do I merely write the number in the line or do I have to include the tag, or the tag and remaining plastic inner insert with my submissions?

You already crossed them over to raw by cracking them out of their holders
