where, and how, would I tell if a slab has been opened and a specimen has been replaced? They're much like the ACG holder......CHEAP/JUNKY, however you wanna look at it...

Yep, they are very evident if cracked open. The urban legend of coin replacement is basically that. Only one known case among the leading brands that I have heard of, the counterfeit first generation PCGS slab.
