Pricing conflict on submission forms.

Why isn't the price structure the same on the on-line submission and the hardcopy submission forms I received in the mail? The hard copy forms show a higher price; which is correct?
For example, my last hard copy submission shows the price of grading for a "total declared value" of
$0 to $100 $14.95
100.01 to 1,000 $15.40
1000.01 to 2,000 $15.85
while the on-line form has
$12.50, $13.50 & $15.00 respectively and further increased based on the number of coins submitted
If the on-line form is correct, why aren't submitters who use the hardcopy form reimbursed for overpaying? I know it is an insignificant amount but I seem to have overpaid on my last submission, that is, if we should be using the on-line prices. So, which is it?

I think you have an older submission form, circa 2000. I believe there is no conflicting info on the newest submission form.
