Proof or prooflike?

I have a coin that I'm not quite sure if it is a proof or a very prooflike "first strike" coin. What number should I put while sending it for grading? The number for proof or prooflike?
Should I write a note in the submission form explaining the situation?
What tools do you use when determining the proof vs. prooflike coin?
