How is it possible to have???

A M/D and D/D on the same coin and in the same area?For instance the Kennedy Half.Lets say the United States of America is MD while the EPluribus Unum is DD.In what sequence would this happen and what would go wrong to create this.

    The Doubled die is created first when the die is created by the die not being in the exact same position between hubbings ( or because of being hubbed by two different hubs with a difference between the hubs). the Machine doubling is created during the striking of the coin when the die bounces or moves laterally during the strike because some of the parts of the press have worked loose and need tightening. It is possible to have machine doubling on a coin struck by a doubled die.
