Is there a site to download a NCS submission form?

No, you have to call them.

That is in their future plans, though. Technologically, both NCS and NGC are a bit behind the curve.

You have to be a member of ANA or NGC Collector Society to submit to NCS.

The first time I used them I just sent them an email and three days later I had all the info and forms I needed. mike
