How does PCGS examine a coin submitted for upgrade?

I’m interested in understanding the flow of work at PCGS. Please check my understanding of what happens when you submit a PCGS-slabbed coin to PCGS for a possible upgrade.
Is the coin reevaluated in the holder?, Or, is the coin cracked out of the holder, put in a flip and set to a grader for evaluation as if it were submitted raw? If the latter is correct, is it true that the grader really doesn’t know if (1) the coin was previously graded and slabbed or (2) submitted raw?
Also, can you submit a PCGS-graded coin for possible upgrade, but request that under no circumstances it be given a lower grade? (I believe that this type of request would be honored for a crossover.)

I'm not sure if it is in a PCGS holder...probably the same process.
But, you can request a minimum grade to be given on the submission form. If the grade above is not achieved, the slab will be cracked....I think this is all correct.
According to the various posts I've seen from Rick and David, coins submitted for Regrade (PCGS coins) are cracked out and run through the system as a raw coin would be. The difference comes at the end. In the event the "new" grade is lower than the "old" grade, the finalizer automatically adjusts the grade so that it is reholder with the previous grade. You do not have to request this--it is part of the regrade process. There is no way to submit a PCGS coin for regrade and keep it the old holder.
