Submitting in original proof holders? Flags?

Could someone direct me to where there is info on submitting Proof State Quarters and whether it is possible to submit the actual sealed proof set to prevent handling damage? I realize there is an issue with labeling with the line numbers, etc. but this seems to be the most practical. When I have submitted Goodacre Dollars in the past, I always submitted them in their original ICG holder as directed by PCGS Help Line. I was also instructed that I could submit multiple identical coins in one tube (e.g. submitting 25 dimes, all 1924 Mercs with the same PCGS #)

Secondly, if I submit some of the Statehood Quarters are they automatically given the "Flag" or is that an additional charge/hoop? How do I request the flags?
