Anyone fill out a ncs form? I don't understand..

The shipping part say's to select the return service prefered.
Ok so I checked registered mail.
Then on the right it has three blanks with the following.
Maximum insured box value:
Maximum actual box value:
Maximum insured day value:

What do they want here I have no clue?

This is part of the form to apply for a submisson user number not a coin submission form.

    It must be an old form. Mine doesn't even have the boxes. It simply asks for a total declared value at the bottom of the coin listing.

    I haven't checked, but you may want to go to www.ncscoin.com to see if you can register or whatever online. At the very least, there'll be a # to call if you run into probs.

    When I sent in my application, I ignored those three blanks and it processed just fine.

    Unless they just added it, there's no way to register online. They do have the phone number online, but it's also in the materials Placid already has.

    I can't help you there. But recently I gave them a call and they informed me that I couldn't send them anything unless it was through a dealer that was registered with them.

    That information is not correct. I'm not a dealer, and I've done several submissions directly to them. An individual can submit if they are a member of the ANA or NGC's club (can't remember what it's called), but you have to go through the application process as Placid is doing right now. Once completed, they assign a submitter number.

    leothelyon I just used the info on their web site and called for a ana member submission packet.
    Unless they changed it a ana or an active member of the collectors society can send in coins once you send in a application.

    Thanks for the heads up. I couldn't remember everything that was said, only that I knew I wasn't a member of whomever they said they take submissions from or through. I've been to that website
    but didn't see the kit. I'll get the kit and go from there. I know a couple of local area ANA members if I need their assistance. I have one coin that I'm worried about, a 43/2-P PCGS66FS Jefferson that has a pinhead cs or black lint up in Jeff's hair. I'd crack it out and try to remove the spot myself but I think I'd rather have them do it, if they can and then have it go from there (NCS) then back to PCGS. This way, I believe, PCGS would give the coin more creditability that it was once certified by PCGS.
