I shoulda just kept it at home.
I am painfully familiar with this problem. Sometimes these proofs grow crud all by themselves. PCGS has nothing to do with it. I used to think they did, but I've done quite a few experiments here with different types of flips and poly bags, as well as just leaving them out exposed to air. I've have more then my share that come out of the packaging pristine and beautiful, and turn to complete crap in a couple weeks.

I don't blame PCGS. I just blame myself for trying to get in on this modern scam. It must be some sort of racket or something where big modern dealers can get the grades they want, and the little guy can't get a break even if they only want it for their own collection. I've learned my lesson, and it's one I already knew. Modern coins belong in albums, not in slabs.

If that were actually true, then I wouldn't have just made the same coin as yours in 69DCAM. I sent in one only, and that's the grade it got. BUT, I made sure I left it out for a while before shipping it off. Also scored a 1992 in 69DCAM on the same invoice.

jtryka,
I want you to know that I completely understand your frustration with the process. The problem has cost me a LOT of money in ruined submissions. Here's an example of what an entire batch of coins came back looking like early on:
That coin was beautiful and nearly flawless when submitted. Every coin was ruined on that submission. I discovered that, in this case, it was crappy flips that were leeching all over the coins.
