1977 Proof Set Post Mint Damage

There was recent discussion about mint errors in proof sets. I've posted this before.

This is obviously damaged. At what step in the process could this have happened? It looks like it was forced into the holder with a screw driver. 🤣

Back in 1977, does anyone know the process of putting these mint and proof sets together? Where it's done? When it's done? How it done? Machine? by hand? The path of the coins take from minting to packaging? Where or how long are the coins stored before packaging? .. etc..
