Best I can tell this was written by T.J. Jankovsky in 2008. I am not sure if it was printed or released in a digital format. Does anyone know if this guide can still be found anywhere?

It is available in a digital format. I do not recall seeing a formal hard copy other than self printed.
IIRC Heritage had some involvement around the time of its original release, possibly in conjunction with an auction of similar material. I do have a copy but I do not know who controls the publication and/or distribution rights so I don't feel comfortable sharing it straight out (work product and things like that still matter to me, interwebz scouring be damned). I haven't checked but it may be available in the Newman Portal?
I looked in the Newman Portal, but could only find references to the guide and not the guide itself. There was a website apparently, but it doesn't seem to be up anymore.
