Grading a high grade coin

Has anyone ever been in a situation wherein you submitted a coin thinking it'd grade say MS66 and it came back higher such as MS68? What happens in this case? Will PCGS bill you for the walkthrough rate?
Or, in the alternative, will your grade be capped at that tier in correlation to the PCGS Price Guide?
Or is most people's experience is that to get that grade, you'd need to go in for the regrade or reconsideration?

You get bumped to the grading fee that corresponds with the grade.
Granted, you get less experienced graders on the lower tiers, so chances of getting a higher grade there are generally considered to be lower.
