Which PCGS form to use, and how many forms?

I submitted this question directly to PCGS, but got a confusing answer, so let me try again here.
At the World's Fair of Money show in Oklahoma next month, I want to submit 12 coins to PCGS. 10 of the coins are from France, one is from the Australia, and one is from the USA.
Note that I am not asking for the coins to be returned to me at the show. I only want to submit the coins at the show. PCGS can then follow its regular grading process and return the coins to me by mail.
For submissions at shows, the PCGS website says I should use the "show submission" PDF form. Do I list all 12 coins on a single show form, even though they are from different countries? Or do I need to prepare three different show forms, one for the coins of each country?

The "show" form only lists 'show service levels' (e.g., "show rarity", "show express", "show", etc.). So, if you do not want to pay a premium to get the coins back at the show, this is not the form for you.
Unless I am missing something, you should use the "U.S." form for the U.S. coin, and the "World" form for the foreign coins.
