How many would get punched before employees notice and line shut down for new dies? or is there automatic detection and shut-down?

I would think that Coins are produced in such a high volume it wouldn't be easy to accurately say how many occur before they notice.
I normally don't care for ATS holders, but I would make an exception on this one.

Wild stuff! As The Dude would say, that shattered reverse really ties the whole thing together.
It appears that this coin has a hole in it as struck at the edge of Sacagawea's forehead above her left eye. I'm also wondering if the wrongly colored bit approximately at the eagle's neck is an embedded piece of the reverse die.
