Just downloaded the .pdf Restoration Service form but in the lower right corner it says the form is not valid after 12/31/2024.
How can I get a current Restoration Service form?

I don't think that would stop me from using the form.
The current PCGS submission form I got from the PCGS booth and used at the FUN show for World Coins had, "form not valid after 6/30/23" on it.

Our team is working to update the forms for this year. You can still use the 2024 forms in the meantime.
