How to fill out 3rd-party grading form?

I might want to submit a 2024 Paris Olympics proof coin to a 3rd-party grader, and I'm a bit confused about how to fill out the submission form. For this question, I am using the IGC submission form as an example, but my question really applies to any 3rd- party form.

The attached image shows the Certificate of Authenticity that came with the coin. On the IGC form, there is an Attributes field. To fill out the IGC form, do I just create my own abbreviation of the information that I want to appear on the slab label? In this case, Olym Paris 2024 Héritage Versailles No 5316?
