Submission Form - Insured Shipping

Hello,
First time that I am going to use the PCGS form to submit some world coins.

On the online page, there is the part that charges the insured shipping based on the declared value and number of coins.

Where is that fee added on the form?

On the Add On Fees, maybe?

    Looks like it will be better to continue using the online form and just do the submit and print it, I just want it to start using the payment method via check or money order versus the credit card option that I used in all my previous coin submissions.

  • On the very first page of the paper submission agreement, the page one you tear off, it has both the domestic and international shipping and insurance rate chart towards the bottom. It breaks it down by number of coins vs. the total declared value to determine the cost.

    Yes, I saw that and also it is $33, but where do you add that on the form?

    In section #9, on the line titled "return shipping fee".
