Has anyone ever seen anything like this? Can you tell me if this is an error or stamped after minting?

Just a common Counterstamp. I have many thanks to a certain someone....
This is my other favorite from my collection:

I got one that is unique like yours, i just got it in change the other day.

May have been a tool check token.
Found it, It looks like the OP's was struck with a roller machine. Much different than a hammer and mallet.
Most except for Sal's is done with a nail style punch.
Maybe it's a prisoners number.
