Need Help Identifying Swiss Medal

I found this on eBay, but I cannot find any information about it anywhere else. Any help on identifying this medal would help.

The reverse translates to the Republic of Ticino.
Did that exist at some point? I've never heard of it.

Ticino is the southernmost canton of Switzerland. https://en.wikipedia.org/wiki/Ticino. Medal likely commemorates The Act of Mediation in 1803 when two cantons combine to form the modern canton of Ticino.
It looks like a modern medal, possibly from 2003, a bicentennial year for that event.
