I can't really tell from here with this picture. Looks like a hit with displaced metal. Can you get a closer picture?

Very likely pmd. Small chance that it's a small strike through that's flipped over and not fully detached. Same value either way.
It came from an Unc roll.
That doesn't matter, the mint does not handle coins gently once produced. Agree PMD.
An Unc roll of 49-D's?
In many cases, if it was a struck thru there would be others in the roll.

Looks like a planchet flaw to me, but that's kind of a catch-all. Specifically it might be a really small peeling lamination but it is pretty grainy in the picture.
