If you thought it was CAM, I’m not sure why you’d submit under guarantee resubmission. That’s the service you use when you think the coin is altered or terribly over graded. If you want the cam designation, you’d have to regrade or recon but I’m not sure if it would even be wise with that holder.
Many collectors pay cam/dcam premiums for coins like that in the old holders, before they started giving out those designations. My advice is to send that to CAC and try to sell as is, if you were planning on selling it. Just because it’s not on the holder doesn’t mean someone won’t pay cam money for it. Based on those pictures, I certainly would.
I have always said and recommended that the grading companies should offer a contact point just to find out why stuff like this happens. I wouldn't mind being charged a fee for the 5 minutes it would take to look up a file in the computer to see what the graders thought when viewing this coin. If 5 minutes were spent by the source, then charge $20 which would earn PCGS $240/hr. I doubt very seriously if the original graders spent that long forming the grade. JMO

Depends on your end goal, but I would send it back for reconsideration where if it upgrades (to the minimum you specify), they will reholder and regrade it. Otherwise you get it back. I would do that before sending it to CAC, unless you'd prefer to just have a bean on it and not necessarily in the highest grade holder.

From my limited experience in trying to get cameos is the lighter shading around the 11 o'clock positions on both the obverse and reverse and the 2 o'clock obverse prevent the coin from receiving a cameo designation. Part of the reason for this strict interpretation is the PCGS price guide value almost doubles for cameo.
Even if the coin were a cameo, CAC would only evaluate whether it was an acceptable PF66. It would ignore the possibility of a cameo.
