PCGS submission question.

On which line of the submission form do I enter the voucher number? I don't see where to do it. I haven't submitted to PCGS directly in a while. TIA.

I put MEMBERSHIP RENEWAL in the "OTHER" box in the last column under service level.
Also have older forms I'd prefer using to save trees, even though prices change, I'll just scratch out the old prices and let the customer service personnel fix it. They like to save trees, too.
