On my last proof sets I have'it to look, to day I finaly find this very rare FS-901. VV estimated somewhere in 1996 as 1% (perhaps). After all years past, in 2016 Herb told me that just .0.25% was believe to be.
This coin it is one which I want to finish the book and also the collection.
I put only two photos: The main marker: the N in EPU, the only one in this year without sherrif, and a full of the Reverse. All pics was took over original package. In 2015, the Mint has 3000 proofs sets in teirs reserve. Herb and Me we find these veriety in all with the observe MM in stege C. How manny they sold after??? No data was publish. For today, IMHO they destryed all the reserve sets. True or Not??? I have no idea.

I've checked about 300 sets many years ago and never found this one.
My luck always seems to run hot and cold though.
Your ID of the RDV-005/Type E Reverse is correct.
