2001-D Buffalo Authentication Question
Has anyone seen this PCGS Buffalo Label before? I don't see anything necessarily wrong with the coin although I'm not an expert at authenticating modern commemoratives. The slab and font on the label appear correct to me, however, I know that the counterfeiters are getting better at faking these.
With that said there are two reasons I have questions. First, I have never seen this particular label before. Second, the barcode is a little wonky. It works, but one time when I scan it, it brings up the correct coin. However, sometimes when I scan it, it brings up a 2016 ASE. There is a small imperfection in the barcode, which could be causing the problem, but should that imperfection also be a cause for concern?
I would appreciate any and all opinions, and I apologize up front for my poor photography skills.

Custom label for a large marketing company.
Thanks @djm I appreciate your input.

Any idea of the name of the marketing company? I plugged in 2001-D Buffalo PCGS and done of the label images look anything like label on the side where the Indian is depicted.
Something similar from the "completed items" at GC.
Edited to add:
Have not been able to identify the source, but a quick eBay search returned the following:
#1
#2
#3
Note #1 - I intentionally excluded the exact match.
Note #2 - To see all of the images in the eBay links, click "see original listing".
Thanks @MetroD, seeing those other examples, especially the one from GC, makes me feel confident the barcode issue is just an anomaly and the coin and slab are legit.
