Welcome to the forum. Everything I see on your nickel is wear and damage in my opinion. The circle on the reverse is damage from a coin counting machine or vending machine, everything else I would attribute to 59 years of circulation.

    Welcome
Nothing there that can’t be attributed to normal wear and tear over over the years.
If it was struck three times, it would affect the whole coin, not just the forehead.
It would cost you over $50.00 to grade that coin which is worth about 5 cents.
