How Do You Establish Provenance for PCGS to Add it to Your Holder when You Submit Coins?

There have been some discussions on provenance lately and I have a question. I have never gotten around to it, but I have some coins that Künker auctioned off a few years ago that came out of the Otto Horn Collection in Germany. They have the original collector's round tag.

What does PCGS require to add provenance to the slab? I assume they just don't take your word for it?

Do you just submit the info with the submittal and they they will send it back?

I have spent a awhile looking at the PCGS website for the info but I can't find how to do this.
