Please help me understand these various PCGS generation of inserts/slabs

I think I have this right. The current, or close to the current generation of PCGS, is this:
Then I see sometimes a generation that doesn't have the series and coin number on the insert:
Now I've come across an insert that doesn't have the series number or coin number and the barcode is on the back:
I've found modern Commemoratives that look like this, yet under the PCGS Slab Museum, it states that for the 4.0 slabs, there are series, including Modern Commemoratives, that exclude insert info. The Ike series is not one of them. Why does this Ike not have the coin and series number on the insert? (Also the "T2" instead of "Type 2" designation is interesting.)
I wonder if this was done for such a short period so as not to warrant inclusion in the PCGS Slab Museum?

Your 1976-D T2 Ike is in a holder that Conder101 has Identified as a type 6.1 holder in his 'PCGS Holder Generations' posts. And can be found as No Type generation (6.0), T1, T2 (6.1) or as TYPE 1, Type 2 (6.2)
Mike Sherman at PCGS put out the 'The PCGS Slab Generations 1986-2016' and ID this as Generations 4.0 as the first blue label slab. With a note on this style of label.
"A very few of the early Blue Label regular issue U.S. coins (mostly modern issues) appeared without the Series and Coin Number. We (at PCGS) believe these were not a regular production generation, but appeared only on a few coins whose Series and Coin numbers had not yet been established."
I have found that the Ike series has many of these label and I never did get a chance to talk to Mike and find out why the Ike's have this label. As the blue label was issued from October 1998 thru February 2002 and the Ike series was well established by that time.
