I can't see anything unusual myself.
It appears to be a Charles III 1/2 escudo from Madrid Spain, which you can identify from the crown over the M on the reverse. The portrait is that of a young Charles, sometimes called a "rat nose". The mature portrait of Charles III on both escudos and reales can start to be seen from 1772.
In trying to blow up the image there does appear to be some sort of possible repunching going on.
Perhaps someone more knowledgeable will weight in.
