Tru View thru holder?

Is a coin always photographed raw for Tru View?
I sent in three coins for reconsideration, all marked for Tru View. One of the coins upgraded and was Tru View photographed. Another coin didn't upgrade and was Tru View photographed, and the third coin didn't upgrade and was not Tru View photographed.
I'm curious as to what the difference was as to enable a photo for the second coin but not the third. Since the second coin didn't upgrade, I'm assuming it wasn't removed from it's holder, yet it was Tru View photographed.
I asked the receptionist at PCGS, (the girl who answered the phone) but did not get a satisfactory explanation.

@PCGSPhoto has stated previously that a coin can be truviewed in its existing holder. Some coin holders have special significance and would prefer not to be lost; rattlers, doilies, regency, etc...

I have one that was a mistake.
I paid for a new holder & TrueView but they re-slabbed it & used the old photo.
You can see the old gasket obscuring part of the rim and it's in a new holder that has a prong type gasket.
They said to send it back to fix the problem but I haven't done it yet because of how busy they are.
