Autograph Special 101

I've subbed many cards, but never autographs. With the current special, is it $15 for an autographed ball or other item that fits into a PSA slab per the guidelines and that's it, or is there another fee to add to this? For example, if I send an autographed Stan Musial ball is it $15 plus applicable fees (shipping/insurance) and that's it? If I send an autographed Phil Simms rookie, is it $15 to slab it or is there another charge I'm missing? The form has alot of 'optional' charges and I'm trying to understand.
I only want the $15 special...no other 'options'.

If you're just authenticating the auto, then no other fees. Something like an upgrade to full letter LOA would be extra.
