What does orange peel surfaces mean... I assume the ripple effect on the surface of the coin, but what is it caused by?

The 1967 SMS DDO quarter is one that is common to find with machine doubling.

Certainly it's possible. A prime example would be the 1967 SMS DDO-001 quarter, as already mentioned. Most of this variety show both hub doubling and machine doubling. One without the machine doubling is unusual. It's also seen on the 1916/16 5c-
