    What does orange peel surfaces mean... I assume the ripple effect on the surface of the coin, but what is it caused by?

    Looks like it might be a touch of machine doubling, it looks like the mint mark shows a touch of it too. ;)

    Yes

    I would guess yours is all machine doubling.

    The 1967 SMS DDO quarter is one that is common to find with machine doubling. ;)

    The 16/16 with machine doubling is much scarcer than the one without.
