Sorry, but it's not an error. It's commonly referred to as a dryer coin The coin gets stuck and tumbles around. This can happen when a coin gets stuck between the drum and outer wall of a commercial dryer. There other scenarios where this can occur as well. It's a damaged coin

Looks like it might be some type of novelty item and not a genuine U.S. coin.
