My guess: $2-$3 on average including the cost of the machinery to seal it and labor when adjusted for the large volume.

Exact same for me as a dealer/collector. Temperature, sharpness of rims, and I'd assume some allowable tolerance down to the fraction of a millimeter by our gasket manufacturer all play a part in this occasionally happening. But I can't ever remember the gentle tapping trick not correcting it, typically for good.

Tapping works for these soft rubber inserts ? Rattlers yes but I would think the soft inserts would stick

I assumed for rattlers you just need to keep turning the holder to keep the coin rolling until it lines up - is tapping necessary for those?

For softer materials, I always understood that tapping or vibration was the way to go.

Just the plastic... they probably buy them by the thousand ... so I'll say 150 bucks per 1000.

Labor is the big expense. You could try your electric toothbrush on the side to get it back to normal.

I strongly believe that the PCGS holder is better than the alternatives, truly a credit to the high tech and creative minds out there in CA.

Electric toothbrush is very effective and saves from whacking the slab on a table.
