Looks like:
Abraded, sanded, and debris removed.
In other word, metal detector find that was cleaned.
Or simply, a cleaned environmentally damaged coin.

It was subjected to high heat. Notice the large raised area behind the portrait on the obverse. That's due to the clad layers starting to separate.
In the grand scheme of things, it really isn't that important as to what caused the damage. It's important to recognize that it didn't leave the mint looking that way
