I found this the other day and was wondering if this could be an original photo card? I can't seem to find another like it online. Any help is greatly appreciated!

This is a 1936 National Chicle fine pen premium. Not worth a great deal but never the less a nice card.

Would this get an MK designation because of the pencil on the back? What if I erased it?

Yes, it should get a MK qualifier. Erasing is considered an alteration and should get Auth-Altered at best.
