Errors on a 1982-S Proof Washington Commem

I acquired this in a collection of 'errors' and 'varieties' I recently bought- the single quotes are because many of the coins were neither. This Washington commem stood out, though. It appears to have a filled die on the obverse, and multiple strike-thru's on the reverse, which I think are related. I find the obverse especially interesting. I had to shoot the photos through the capsule, because I had trouble opening it and didn't want to risk damaging the coin. Am I correct in my assessment? I'm more of a variety guy than an error guy so any feedback is appreciated.
