The coins are first struck, then are placed through an entirely different machine, a castaing machine, to impart the edge lettering. Only proofs are struck with a lettered collar. So, nothing could fall off of the edge and onto a die to be struck in because anything off the edge that falls would not have lettering. -

It could be, if used to apply a second strike (80% or more off-center) on a genuine 1968-S quarter. There is a similar torched nickel die out there that was utilized to apply false secondary off-center strikes on 1968-era nickels. One way to identify these is that the maker did not have a corresponding reverse die to use. -
