As for #1 above, I can confirm it's not always based on submission date because I received several red boxes for orders submitted in May that I received back in July.1 -

Gently remove all the coin doctor residue and it has a chance. I've seen straight-graded coins with much worse damage than that.2 -

I do appreciate the really old and lower-grade coins that sticker. They took some punches but no one used them as a science project.2
