There's no small flat rate box size either. I typically will select priority mail/package and put in the dimensions of the box. Then enter the weight and typically it comes within a few cents of the flat rate charge(making sure it's over and not under).
If its an eBay sale, instead of using the paypal option you can use the ebay shipping option. I see small, medium, large and package when I do it this way.

Where do you find the "Ebay shipping option"? I go to Ebay and click "Print Shipping Label", and it takes me to Paypal.
When I do it on eBay it defaults to their service, I remember when they first rolled it out I had to click on an agreemnt to allow the billing to passed to my paypal accont.. it's actually easier and quicker than doing it through paypal (it eliminates a step for one and the tracking info is right in your sold items page as another). I just started Paypal shipping this week and just got more familiar with it today. I guess I need to get used to Ebay shipping now. Looks like it's worth it.
Just to piggy back a little on Ken's answer-there are other options in that same area you may or may not want to change such as excluding shipping to some continents (if you always choose to not ship to Asia for example), and you can also set your flat shipping rate, or promotional rates etc;
There's a ton of stuff in the detailed settings that most of us don't even realize.
