A Few Questions About Reviews

I'm thinking about taking advantage of the review special and sending some cards in for review. I haven't submitted anything since the 1/2 point system was implemented and had a few questions I hope to get answers for before deciding:

1) I read through historical posts and saw some posts stating PSA reholders all cards they review regardless of whether they get a bump. Is that true?
2) I also read where someone said PSA provides post-it notes on cards that fail to bump explaining why they don't bump. Is that true? They did used to do this when I'd send cards back when I felt they were undergraded but I would be surprised if they'd do this for mainstream bump submissions.
3) It seems that about 15% is the typical average for cards getting at least a half point bump. Is this still a typical hit rate (for vintage postwar)?
4) Centering seems to be the number 1 criteria for bumping with strong corners and print being a close 2nd and 3rd. How do you rank in order of importance?
