Sub help needed

I am currently filling out my first sub form and I do not know what to put for Dealer/Member and Dealer/Member #. Can someone tell me what this means?

Are you filling out the online form or paper form from your member kit?

Either way, it should be your 6 digit account number.

it is the forms they sent me with my package so the member name is just my online memeber name and where can i find that number?

Log into the sub center, and click on print a copy and all that info will be there for you.

The easiest thing for you to do is click on submission center. Enter your email address and cu password and it will bring up the online form with all your info on it including your account number.

You can also click on "My Account" at the top of this website for your info.

Filling out the sub online is very easy to do.

When it comes time to print... just print pages 1-4 and be sure to print copies for yourself.

Yup on can clik 'my account' too, that would be the fastest.

Just in case the OP is wondering where 'my account' is, it is not on this page but on the home page.

Or you coulf look at our Welcome Letter or Voucher. We print your account/Member number on both.
It is also on the copy of your invoice for the membership uncer Customer #.
